Navigating the transition - from science-based education to practice

Discussion and research on the challenges with provision of patient-centred pharmaceutical care in the face of scientific innovation and progress are numerous. Explorations in this area include practicing the current knowledge of pharmacogenomics, to new technological innovations for enhanced medication compliance, and to designing academic strategies all geared towards the final target of professional excellence. With this aim of bridging the gap from “science” to “practice,” it is vital to have a close examination of pharmacy students and young pharmacists settling into their new roles. Guidance should be provided to help young pharmacists navigate through the dissonance as they transition from the ideal world of pharmacy training to professional practice. This session intends to prepare students and young pharmacy graduates in personalising their individual approaches to their future practice, through an open debate between themselves and experienced healthcare workers in the field. Perhaps, just as educators intend to situate science within the patient-centric approach, they too, should consider “personalised education” to create the optimal environment for the seed of excellence.

At the conclusion of this session, participants will be able to:

  1. Describe the challenges and opportunities involved in implementing scientific innovation to practice from the model academic environment to young professionals in their fields.
  2. List opportunities for creating progressive personalised education from the perspectives of educators, pharmacy students, practising pharmacists and industry leaders.
  3. Identify the features of high-impact tools in the provision of patient-centred pharmaceutical care paving the path towards self-directed professional excellence.
  4. Evaluate new tools geared for optimising pharmaceutical care in practice (i.e. technological innovations) for important key features such as usability, reliability, engagement of patient accountability, and comprehensiveness in addressing targeted issues.
  5. Describe ways to become more self-directed in shaping professional excellence in their own field of practice (i.e. emotional intelligence management).
